Before you visit the branch
A name change in a bank account is really a KYC update. The smoother your documents match, the faster the branch can approve it. Work in this order:
- Get proof of the new name. After marriage this is usually the marriage certificate. For other changes, a gazette notification or an affidavit is typical.
- Update Aadhaar and PAN to the new name if you can. Banks verify KYC against these.
- Fill the application above and print it.
- Sign twice — old name and new name — and attach self-attested copies.
- Carry originals for verification and a recent photograph.
Document checklist
| Situation | Commonly requested |
|---|---|
| Name change after marriage | Marriage certificate; Aadhaar/PAN with new name; photograph; existing passbook/cheque book |
| Spelling correction | Aadhaar and/or PAN showing correct spelling; passbook |
| Other legal name change | Gazette notification or affidavit; updated KYC; photograph |
Banks set their own checklists under RBI’s KYC rules; your branch may ask for more or fewer documents.
After the change: update these too
- Debit card and cheque book (request new ones in the new name).
- UPI apps and linked mobile wallets — the displayed name updates from the bank.
- Nominee and joint-holder records, if any.
- Fixed deposits, PPF/SSY accounts and lockers at the same bank — mention them in the application or ask the branch to update all linked accounts.
- Employer salary records and insurance policies, so payments match.
Wording tips
Keep the subject line specific (“Request for change of name in bank account after marriage”). Mention the account number in the first line. List each attached document. Avoid long explanations — the branch needs facts, not reasons. If you hold more than one account at the same bank, list all account numbers in one letter.
